ADAM chose 0.25% topical finasteride because it is the concentration currently supported by the largest published randomized clinical trial (Piraccini et al.). This trial demonstrated meaningful hair growth with substantially lower systemic finasteride exposure (more than 100-fold lower plasma concentrations) than a 1 mg oral tablet. Current evidence does not prove that 0.25% is the universally "best" concentration, nor that lower concentrations are equally effective. Concentration alone does not determine systemic absorption—formulation, application volume, scalp surface area, and individual skin biology also heavily influence drug delivery. Because of this, "2.5 mg applied" does not mean "2.5 mg absorbed." Our 0.25% finasteride and 5% minoxidil formulation represents an evidence-informed decision to anchor our treatment to robust clinical data, rather than relying on theoretical marketing claims.
